We need this form returned to us no less than 14 days prior to the date you depart to your summer camp. We will return it to no later than 7 days before camp. We will verify the information, and make a note of any adults that are not qualified to attend summer camp

NOTE: It takes 3-5 days for the average results to come in. Plan accordingly.
